In 1966, Williams and Charlie Tucker were working on the scaffolding when one of the boards on which Williams was standing slipped unexpectedly. Williams fell four hundred feet to his death, striking the head of Stonewall Jackson on the way.

Other carving crew casualties included Thomas Melvin Kennedy in 1927 and Nelson Wilborn in 1971.
