Thomas Melvin Kennedy became the first fatality of the carving project in 1927 when he was thrown off the scaffolding by a falling rock slab. A plaque in memory of Kennedy can be found at the Memorial Lawn area near Memorial Hall.

Later carving crew casualties included Howard Williams in 1966 and Nelson Wilborn in 1971.
